How to Answer Assessment Questions and Upload Evidence
This guide walks you through the core process of completing an assessment in Gradum.io. You will learn how to navigate to specific assessment topics, select maturity levels, provide context through notes, and validate your answers by uploading supporting evidence documents.
Prerequisites
- You must be logged into your Gradum account.
- You must have an active assessment assigned to your user profile.
- Ensure you have your evidence files (PDF, DOCX, etc.) ready on your computer.
Step-by-Step Guide

Select the Assessment Aspect
From the "Assessment Details" dashboard, scroll down to the Aspect Progress section. Locate the specific topic you wish to address (e.g., "Purpose / Policies: Purpose") and click the blue Start button next to it to begin the questionnaire. -
Select Maturity Level
Read the question displayed at the top of the screen carefully. Under the "Standard: Maturity" dropdown menu, select the option that best reflects your organization's current status (e.g., Defined Processes). This establishes the baseline score for this specific requirement. -
Add Contextual Notes
In the "Notes" text area, type any relevant comments or explanations. This provides auditors or reviewers with necessary context regarding why a specific maturity level was chosen. -
Upload Evidence Documents
To support your answer, scroll down to the "Evidence Documents" section and click Add Files.- A file browser window will open. Select the relevant document from your computer (e.g., an invoice or policy PDF).
- Click Open to upload the file.
- Verify that the file name appears in the list to ensure it was attached successfully.

Save and Proceed
Once you have selected your answer, added notes, and attached evidence, click the Next Question button in the top right corner. This saves your current progress and advances you to the next question in the module.
Tips / Outcome
After clicking Next Question, your progress for that section will update automatically. You can always return to previous questions using the "Previous Question" button or the navigation menu if you need to update your evidence or notes later.
